Abstract

Nephrotic syndrome (NS) is one of the most common glomerular diseases that affect children. Hypovolemic crisis is one of the complications of NS. The clinical manifestations are tachycardia, cold extremities, poor capillary refill, and moderate to severe abdominal pain, but A 17-year-old female patient was diagnosed as NS 13 years ago, who showed no response to steroid treatment and various immunosuppressants such as cyclosporine A, mycophenolate mofetil, and tacrolimus. She was hospitalized due to general edema and weight gain of 10 kg. On 6th day of admission, she complained of squeezing chest pain, tachycardia (150 per minute) and dyspnea, and therefore electrocardiogram (ECG), chest X-ray, laboratory examinations were performed, which showed sinus tachycardia, and serum albumin 1.0 g/dL, proteinuria 4+, urinary potassium / potassium + sodium ratio 0.73, and pleural effusion on both sides. We diagnosed the patient as having hypovolemic crisis and intravenous albumin replacement was administered, which resulted in the resolution of chest pain and tachycardia. If a patient with NS shows chest pain, acute coronary syndrome, deep vein thrombosis or pulmonary embolism should be suspected. However, there has been no report on the patient presenting with chest pain as a symptom of hypovolemic crisis, because it often accompanies abdominal pain. Reporting this case experience, when an NS patient appeals chest pain, we should think about the possibility of hypovolemic crisis, and volume expansion will be a crucial emergency treatment before diagnosing other conditions causing chest pain in NS.
